Hospitalization


For hospitalization you must:

1. In the clinic at the place of residence (at the local surgeon or pediatrician) receive a referral for hospitalization in the form 057/у at the Children’s State Hospital of St.Vladimir indicating the exact diagnosis, which is written on the doctor’s conclusion whom you were at the consultation. Form 057/у-04 must include the stamp of the organization issuing it.

2. If it is not possible to receive a referral form 057/y, to apply for a free planned hospitalization, please visit https://МСЗ.рф or the “Stolitsa zdorovia” hotline by phone: +7 (495) 587-70-88. When contacting, indicate that you want to receive treatment at the Children’s State Hospital of St.Vladimir.

The hot line “Moskva stolitsa zdorovia” helps nonresident patients to receive information about medical care in Moscow hospitals for free according to the compulsory medical insurance policy.

When contacting the hotline, a personal curator will be assigned to you.
The decision on the possibility and timing of planned hospitalization in Moscow clinics for nonresident citizens insured in the compulsory medical insurance system is made by medical organizations taking into account the evidence, the capabilities of the medical organization and the formed waiting lists for the relevant profile.

3. Prepare the following documents:

  • referral for hospitalization 057/у-04,
  • insurance policy and birth certificate of your child,
  • passport of one of the parents,
  • conclusion of the specialist from whom you were in consultation

4. With these documents, on any working day from 10:00 to 16:00, come to the 5th building, room 1.1, room 1.3 to receive a referral for hospitalization or send these documents to the department’s e-mail.

A referral for to hospitalization is a document that indicates the day, place, time of hospitalization and a list of examinations.


You may change the date of planned hospitalization to the clinic on the foolowing grounds:

If a child is ill and cannot be hospitalized within the indicated period, you can call +7 (499) 268-84-62 (please note that operators speak only Russian) from 09:00 to 16:00 or write an e-mail and change the hospitalization date.

The list of documents and analyzes required for planned hospitalization in the Pediatrics:

  • Birth certificate of a child, children from 14 years old need to present a passport (original and its copy);
  • Compulsory health insurance policy (original and its copy);
  • Referral for hospitalization (form No. 057/у-04);
  • Extract from medical documentation, indicating the results of laboratory, instrumental and other types of studies on the patient’s disease profile, information on past illnesses (form 027/y);
  • Medical certificate (prophylactic vaccination card form No. 063/y), indicating the results of on tuberculin diagnostics;
  • Certificate of lack of contact with infectious patients during the last 21 days at the place of residence (valid for 3 days);
  • Certificate of lack of contact with infectious patients during the last 21 days at the place of study or at a child care institution (valid for 3 days);
  • Clinical blood test (red blood cells, hemoglobin, platelets, white blood cells with the formula, ESR), (valid up to 20 days);
  • General urine test (valid up to 20 days);
  • Electrocardiography (limitation period up to 30 days);
  • Laboratory analysis of feces for helminths eggs and protozoa (valid for up to 20 days);
  • Laboratory analysis of feces for the intestinal group (for children up to 2 years old inclusive) (valid up to 14 days);

  • Birth certificate of a child, children from 14 years old need to present a passport (original and its copy);
  • Compulsory health insurance policy (original and its copy);
  • Referral for hospitalization (form No. 057/у-04);
  • Extract from medical documentation, indicating the results of laboratory, instrumental and other types of studies on the patient’s disease profile, information on past illnesses (form 027/y);
  • Medical certificate (preventive vaccination card form No. 063/у) on vaccinating a child, indicating the results of tuberculin diagnostics;
  • Certificate of lack of contact with infectious patients during the last 21 days at the place of residence (valid for 3 days);
  • Certificate of lack of contact with infectious patients during the last 21 days at the place of study or at a child care institution (valid for 3 days);
  • Clinical blood test (limitation up to 14 days);
  • General urine test (up to 14 days old);
  • Laboratory analysis of feces for helminths eggs and protozoa, scraping for enterobiosis (limitation up to 20 days);
  • Laboratory analysis of feces for the intestinal group (for children under 2 years of age, up to 14 days);
  • Biochemical analysis of blood: glucose, total bilirubin and its fractions, AST, ALT, urea, creatinine, alkaline phosphatase, protein (prescription up to 14 days);
  • Electrocardiography (limitation up to 30 days. If there are changes on the ECG, a cardiologist’s opinion on the possibility of surgical intervention is mandatory);
  • ELISA Blood test for syphilis (total antibodies) (limitation up to 3 months);
  • Blood test for hepatitis B and C markers (up to 3 months old);
  • Blood test for HIV infection (up to 3 months);
  • Study of the blood coagulation system (prothrombin time, thrombin time, fibrinogen, APTT; (prescription up to 14 days);
  • Determination of blood type, Rh factor;
  • The opinion of the dentist on the rehabilitation of the oral cavity (limitation up to 30 days);
  • The conclusion of the pediatrician about the absence of contraindications for surgical treatment (limitation up to 10 days).

When hospitalized in a planned form in the Department of Otorhinolaryngology and Maxillofacial surgery, the following are additionally provided:

  • A smear from the nose and throat on BL (diphtheria) (valid for 20 days).

The list of documents and analyzes necessary for planned hospitalization in the Country Department (Branch):

  • Birth certificate of a child, children from 14 years old need to present a passport (original and its copies of pages 1, 2 and 5);
  • Passport of the parent (legal representative) at whose address the child under 14 years of age is registered (original and its copies of pages 1, 2, 5 and 17);
  • Compulsory health insurance policy (original and its copy);
  • Referral for hospitalization (form No. 057/у-04, valid for no more than 20 days from the date of issue until the date of hospitalization);
  • Extract from the medical documentation from the pediatrician, indicating the results of laboratory, instrumental and other types of studies on the patient’s disease profile, information about the illness (form 027/y);
  • Medical certificate (preventive vaccination card form No. 063 / у) on vaccinating a child, indicating the results of tuberculin diagnostics;
  • Certificate of lack of contact with infectious patients during the last 21 days at the place of residence (valid for 3 days);
  • Certificate of lack of contact with infectious patients during the last 21 days at the place of study or at a child care institution (valid for 3 days);
  • In the presence of concomitant diseases, a specialist’s opinion on the absence of contraindications for exercise therapy and physiotherapy;
  • Clinical blood test (red blood cells, hemoglobin, platelets, white blood cells with the formula, ESR) (valid up to 20 days);
  • General urine test (valid up to 20 days);
  • Laboratory analysis of feces for helminth eggs and protozoa (valid for up to 20 days);
  • Scraping for enterobiosis (valid up to 20 days);
  • Laboratory analysis of feces for the intestinal group (for children up to 2 years old inclusive) (valid up to 14 days);
  • Electrocardiography with a conclusion (limitation period up to 30 days).

Legal representatives of minors hospitalized with children should have:

  • Identity document of legal representative - passport (original and its copy);
  • Compulsory health insurance policy (original and its copy);
  • Measles vaccine information or measles antibody titer information;
  • The result of fluorography (up to 1 year);
  • Laboratory analysis of feces for the intestinal group (for parents of children under 2 years of age, up to 14 days);
  • Laboratory analysis of feces for helminths eggs and prozoonosis, scraping for enterobiosis (limitation up to 20 days).

Please, note:

  • There may be a sequence for planned hospitalization of a child, but hospitalization is carried out at the optimum time.
  • The Children’s State Hospital of St.Vladimir does not pay for travel expenses and does not have a hostel for accompanying persons.
  • One of the parents, another family member or legal representative is given the right to stay with the child in the department if the necessary tests and certificates are available.
  • One of the parents of a child under 4 years old is provided with a bed and meals in a specialized department free of charge. Hospitalization of one of the parents with a child older than 4 years is carried out only for medical reasons and is decided by the head of the department in each case, subject to the provision of medical documentation and the results of laboratory diagnostic and instrumental studies.
  • Parents have the right to improve hospitalization conditions at their own expense (according to the price list). Improved hospitalization conditions include the provision of one of the parents, subject to availability, a single room or a superior room.
  • The regime of joint stay of a legal representative with a minor in a hospital may change during the period of restrictive (sanitary and anti-epidemic) measures for infectious diseases.
  • For planning hospitalization you need to have second pair of shoes, toiletries, pajamas or a tracksuit (bathrobe).
