1. General regulations
1.1. The Internal Regulations of the Children’s State Hospital of St. Vladimir (hereinafter - the Rules) are approved by the chief physician of the Children’s State Hospital of St. Vladimir and are the legal document developed in accordance with the requirements established by the Federal Act of November 21, 2011 No.323-ФЗ “On the Basics of Protecting the Health of Citizens of the Russian Federation”, Federal Act of March 30, 1999 No.52-ФЗ “On Sanitary and epidemiological welfare of the population”, Sanitary and epidemiological requirements for organizations engaged in medical activities (SanPiN 188.8.131.5230-10), approved by Decree of the Chief State Sanitary Doctor of the Russian Federation of May 18, 2010 No.58, other regulatory and of the Russian Federation in the field of health.
1.2. The internal regulations are freely available for patients, their legal representatives and visitors to the hospital. The patient (his legal representative or visitor) must familiarize himself with the internal regulations on admission to the hospital (visiting the hospital).
1.3. The rules are mandatory for employees of the Children’s State Hospital of St. Vladimir, their legal representatives and visitors to the hospital.
2. Rules for hospitalization and discharge from the hospital
2.1. Children from the neonatal period to 17 years of age 11 months and 29 days are hospitalized in the in-patient hospital. Newborn children are hospitalized only in neonatal units.
2.2. In a planned manner, minors under the age of 15 are hospitalized in the hospital only with the written consent and accompanied by legal representatives.
2.3. A referral for planned hospitalization (hereinafter referred to as a Refferal) is issued at the Planned Hospitalization Department of the Children’s State Hospital of St. Vladimir (Building No.5), including after consultation in the Consultative and Diagnostic Department of the Children’s State Hospital of St. Vladimir.
2.4. For hospitalization in a planned manner it is necessary to provide:
- referral for planned hospitalization issued by the Children’s State Hospital of St. Vladimir;
- referral for hospitalization in the form of No.057/у-04;
- passport of the legal representative;
- in the case of patient care by a third party, the passport of the caregiver and the power of attorney drawn up in accordance with the requirements of the Civil Code of the Russian Federation to represent the interests of the legal representative / patient, indicating the right of the authorized person to accompany the patient, access to information constituting medical confidentiality;
- birth certificate of a patient under the age of 14 years or a passport for minors over 14 years of age;
- compulsory medical insurance policy (in case of round-the-clock care the child’s representative is provided an additional policy);
- certificate of patient disability (if any);
- medical records of the patient and the person intending to provide around-the-clock care in a hospital:
if available: expert opinion of the Children’s State Hospital of St. Vladimir, extract from an outpatient card, extracts from hospitals, etc .;
medical certificates, as well as the results of instrumental, laboratory diagnostic and other types of studies indicated in the Plan of admission to a planned hospitalization in the Children’s State Hospital of St. Vladimir, including in accordance with the requirements of the Order of the Moscow Healthcare Department dated July 25, 2018 No.500 “ On approval of the regulations for the direction of children's the population for hospitalization in medical organizations of the state healthcare system of the city of Moscow for the provision of specialized medical care (except for high-tech, as well as medical care in the profiles of Phthisiology, Dermatovenerology, Psychiatry, Narcology) in a round-the-clock hospital in a planned form ".
2.5. Planned hospitalization of patients occurs strictly on the day and time specified in the referral.
2.6. Denial of planned hospitalization is possible in the following cases:
- late appearance for hospitalization;
- lack of the necessary documents listed in the Plan for a planned hospitalization;
- lack of one or more results of instrumental, laboratory diagnostic and other types of studies listed in the Referral;
- inconsistency of the results of analyzes and surveys with the expiration dates established for them;
- child’s illness with an infectious disease;
- if after the transferred (including infectious) disease, less than the period recommended by regulatory documents has passed;
- the presence of contact with infectious patients in a child care institution and at home and the absence of a document confirming that the child has had this disease;
- the presence of changes in analyzes or electrocardiogram data;
- lack of accompaniment of parents (legal representatives).
2.7. In case of emergency hospitalization, the information available at the time of hospitalization is recorded in the medical record of the inpatient. Legal representatives, relatives or proxies are required to provide the documents specified in clause 2.4. Rules within 24 hours from the moment of emergency hospitalization. In case of failure to provide these documents, which resulted in a refusal to pay for the medical assistance provided by insurance medical organizations, the Children’s State Hospital of St. Vladimir has the right to recover the costs of paying for treatment to the legal representatives of the patient in court.
2.8. When a patient is admitted to inpatient treatment as part of the provision of paid medical services on the basis of Decree of the Government of the Russian Federation dated October 4, 2012 No.1006 “On approval of the Rules for the provision of paid medical services by medical organizations”, a document confirming payment under a contract for the provision of paid medical services, or the corresponding letter of guarantee.
2.9. Upon admission to inpatient treatment, the patient should have with him personal hygiene items, a second pair of shoes. It is not recommended to bring and transfer valuables. Administration the Children’s State Hospital of St. Vladimir does not ensure the preservation of valuable things (including jewelry, electronic devices and other) and in case of their loss is not responsible.
2.10. The discharge of children from the hospital is carried out daily, except Saturday, Sunday and non-working holidays from 10.00 to 12.00.
2.11. At discharge, the legal representative of the child or a person with a power of attorney to represent the interests of the legal representative of the child must be present, indicating the right of the authorized person to accompany the child at discharge, signing all the documents necessary for discharge, access to information constituting medical confidentiality.
2.12. Documents required at patient discharge:
- patient's birth certificate (passport for minors over 14 years old)
- passport of the legal representative (or power of attorney for representation of interests from the legal representative indicating the range of rights in accordance with clause 2.11 of the Rules);
- compulsory health insurance policy.
3. Patient visits
3.1. Patients who are hospitalized in the hospital are visited at places specified by the administration of the departments daily from 4 p.m. to 6 p.m. On weekends and holidays from 11-00 to 13-00 hours and from 16-00 to 18-00. Visiting patients on days and hours not specified in the Rules is allowed only with the permission of the department head or doctor on duty.
3.2. Entrance for visitors to medical buildings is open from 07-00 to 20-00.
3.3. Visiting patients in infectious and boxed rooms is not allowed.
3.4. To visit patients in a hospital on bed rest, at the same time no more than one person is allowed.
3.5. Based on Art. 13 of Federal Act dated November 21, 2011 No. 323-ФЗ “On the Basics of Protecting the Health of Citizens of the Russian Federation”, visitors to patients aged 15 years and older are allowed only with written consent to visit the patient himself aged 15 years and older; Visitors who are not legal representatives of the patient under the age of 15 years are allowed only with the written consent to visit the patient given by the legal representatives of the patient under the age of 15 years.
3.6. Consent specified in clause 3.5. The rules, given in writing, are written in the name of the head of the department in which the patient is being treated, indicating a document confirming the status of the legal representative of the child (if consent is given by the legal representative) and the person or circle of persons who may be allowed to visit (the surname, name and patronymic (if available) of the visitor are indicated in full).
3.7. Minor visitors are allowed only accompanied by the legal representative of the child being treated.
3.8. A pass to visit is issued by the senior nurse of the department in which the patient is located. Patients and their visitors are prohibited from free movement in the wards, boxes and wards of the building, not indicated in the pass.
3.9. The visit is carried out strictly in second pair of shoes or shoe covers, outerwear is rented in the wardrobe.
3.10. Persons with signs of alcoholic or other type of intoxication are not allowed in the department.
3.11. In order to prevent the spread of infectious diseases, it is forbidden to visit patients with signs of respiratory infections. At the request of the attending physician, department head or senior nurse of the department, a visitor with signs of acute respiratory viral infections should immediately leave the department.
3.12. Walking around the hospital is possible only with the teacher or nurse, with parents, but only with the permission of the attending physician or head of the department. During walks with children it is strictly forbidden to leave the hospital.
3.13. Rules of entry to the territory of the Children’s State Hospital of St. Vladimir:
3.13.1. Entrance to the territory of the hospital of motor transport, with the exception of ambulance vehicles and vehicles of operational services is carried out exclusively by passes.
3.13.2. The administration of the hospital, on the basis of the order of the Moscow Healthcare Department dated December 31, 2015 No.1156, provides for the entry of disabled vehicles (strictly with a special sign), cars with a permit from the Moscow Healthcare Department, and a pass from the Children’s State Hospital of St. Vladimir, vehicles serving citizens with disabilities ("Social taxi").
3.13.3. Cars with patients who are taken to the hospital on their own for emergency medical care are allowed to enter.
3.13.4. Cars may be allowed to enter for discharge of patients who are not disabled, but who are experiencing difficulties of independent movement due to their health, is allowed only at the request of the head nurse of the department upon presentation of an extract from the medical history signed by the head of the department in which the patient was being treated.
3.13.5. Entrance to the territory of the Children’s State Hospital of St. Vladimir cars not listed in p.p. 3.13.1.-3.13.4. of these Rules is prohibited.
3.13.6. Parking of vehicles that delivered patients in the hospital during the patient's in-patient treatment is strictly prohibited.
4. Conditions for hospitalization of persons, who take care about children
4.1. One of the parents, another family member or other legal representative is given the right to stay together with the child for free in the hospital while providing him with in-patient medical care during the entire treatment period, regardless of the child's age, in order to care for the patient.
4.2. When staying with a child for free in accordance with clause 4.1. Of the Rules, the person caring for the child, if possible, is provided with a bed. Free meals for a joint stay with a child of 4 years and older in the absence of medical evidence confirmed by the medical committee for a joint stay is not provided.
4.3. When staying with a child until they reach the age of 4 years, and with a child older than this age, if there are medical indications confirmed by the conclusion of the medical commission of the hospital, parents or their representatives are provided with a bed and meals free of charge.
4.4. In the absence of medical indications confirmed by the medical commission for a joint stay with a child of 4 years of age or older, with persons wishing to take care of the child around the clock, with their consent, an agreement may be concluded for the provision of paid services, including the provision of meals and sleeping places.
4.5. Patient care includes feeding, changing clothes, sanitary and hygienic services, medical procedures, assistance with taking medications and other actions that contribute to the implementation of treatment tactics determined by the attending physician.
4.6. When staying with the patient, the legal representatives of the child, in cases of emergency, can leave the department or hospital (for paperwork for the child, solving registration issues, etc.), coordinating the time and duration of absence with the patient’s doctor or head of the department, informing them in a timely manner this is the nurse on duty.
4.7. A parent or a representative shall inform the attending physician (senior nurse of the department) about the need to issue a certificate of incapacity for work on the first day of hospital stay.
5. Rules for receiving packages. Transfer and storage of products in departments.
5.1. Packages are received in the lobby of the hospital’s medical buildings daily from 08-00 to 19-00.
5.2. Packages are allowed to be transmitted in plastic bags with the name, patient name, ward number.
5.3. Items allowed for transfer to patients and their legal representatives located in hospital departments:
- hygiene items (toothbrush, toothpaste, soap, comb, toilet paper, dry and wet wipes, diapers);
- the necessary minimum of clothes;
- the necessary minimum utensils - a cup, a spoon.
- washable toys, board games, pencils, felt-tip pens, paper, books.
5.4. Products allowed for transfer:
5.5. In all cases, the transfer of products must be agreed with the attending physician. Given the nature of the disease, dietary restrictions may be provided for the patient.
5.6. Every day, the on-duty nurse of the department checks compliance with the storage rules and expiration dates of food products stored in the department’s refrigerators and bedside tables.
5.7. Expired food products (storage), stored without packaging, without specifying the name of the patient, as well as having signs of spoilage, are removed to food waste without warning patients and their legal representatives.
5.8. Forbidden to transfers to patients:
- any products that are not included in the list of permitted products specified in clause 5.4. Rules
- alcoholic (alcohol-containing) products, tobacco products, chewing gum;
- soft toys, playing cards, other gambling;
- sharp objects: knives, scissors, needles, knitting needles, forks, etc .;
- electric heaters, boilers;
- matches, lighters;
- bulky items (including prams);
- toxic, poisonous, traumatic, explosive objects.
6. General rules of stay in the Children’s State Hospital of St. Vladimir.
6.1. Patients, their legal representatives, visitors should be careful about the property of the hospital. Damage caused to the property of the institution is subject to compensation in full by the person who caused the harm or his legal representative.
6.2. Noisy games and loud conversation, running movements are not allowed in the departments. Telephone sets should be used in silent mode during the entire stay in the hospital.
6.3. Phones are charged with the permission of the ward nurse of the department in specially designated places inaccessible to children and provided with a closed multi-socket extension cord.
6.4. Throughout the hospital, including wards, corridors and toilets, cleanliness must be kept.
6.5. Be polite with the medical and nursing staff of the hospital, as well as other patients and visitors, and avoid rudeness. In case of a dispute and / or conflict, act in accordance with the procedure established by these Rules.
6.6. In accordance with paragraph. 2 h. 1 Article. 12 of the Federal Act of February 23, 2013 No. 15-ФЗ “On the protection of the health of citizens from the effects of surrounding tobacco smoke and the consequences of tobacco consumption” to prevent the effects of surrounding tobacco smoke on human health, smoking is prohibited throughout the institution, including premises, street and park areas.
6.7. On the territory of the hospital "prohibited the conduct of unauthorized photo and video.
6.8. Patients and their legal representatives providing care must:
6.8.1. Comply with the Internal Rules of the Children’s State Hospital of St. Vladimir;
6.8.2. Strictly follow the instructions of the attending physician, the recommendations and requests of medical personnel, including observing the medical-protective regime and the diet prescribed by the attending physician;
6.8.3. Promptly inform the medical staff on deterioration of health.
6.8.4. Observe the daily routine established in the hospital, given in paragraph 7 of these Rules;
6.8.5. Observe fire safety regulations. If fire sources or other sources threatening public safety are discovered, immediately inform the medical or other personnel of the institution.
6.8.6. Observe sanitary and hygienic standards for the use of household communications (shower, bathroom, refrigerator)
6.8.7. Ensure timely appearance at the doctor’s office or at the treatment and diagnostic procedure at the time indicated by the medical staff.
6.8.8. To get acquainted and sign in accordance with the established procedure consent and/or refusals relating directly to the provision of medical care to a patient, provided for by the current legislation of the Russian Federation in the field of healthcare, by local regulatory acts of the Children’s State Hospital of St. Vladimir.
6.9. Patients and legal representatives caring for patients are prohibited from:
6.9.1. Occupy beds designed for patients;
6.9.2. Put children on beds for adults;
6.9.3. Keep outer clothing and shoes in the room, and personal items (clothing, plastic bags, things for the child, etc.) - on the bedside tables, chairs, beds;
6.9.4. Store products in places not intended for this purpose;
6.9.5. Store, take, and distribute medications not prescribed by your doctor. Replace medications prescribed by your doctor;
6.9.6. Leave the department without permission;
6.9.7. To leave the infectious box or ward without the permission of the ward nurse;
6.9.8. Take parcels for other patients;
6.9.9. Use the telephone of the department for personal purposes;
6.9.10. Wash and dry clothes;
6.9.11. Store baby strollers in the department;
6.9.12. Use in the department with boilers, electric heaters, electric kettles;
6.9.13. Install personal household electrical equipment (TV, etc.) without the consent of the head or senior nurse of the department.
6.9.14. To smoke and drink alcohol on the territory and in the premises of the hospital.
7. The daily routine in the departments
7.1. General daily routine:
7.2. Depending on the branch profile, the age of the children (newborns, infants and young children, adolescents), changes in the daily routine are allowed.
7.3. The time for carrying out diagnostic studies, medical procedures is determined according to the appointments of the attending physician, the schedule of diagnostic departments (surgeries), and medical specialists.
8. Providing information
8.1. Providing information on the patient’s health status, including information on the results of a medical examination, the presence of a disease, the diagnosis and prognosis of the development of the disease, methods of providing medical care, the associated risk, possible types of medical intervention, its consequences and the results of medical care (hereinafter referred to as information) is carried out in accordance with the requirements of the Federal Act of November 21, 2011 No. 323-ФЗ “On the Basics of Protecting the Health of Citizens in the Russian Federation”.
8.2. Information is provided by the attending physician or other medical personnel who are directly involved in the medical examination and treatment of the patient personally. Telephone information is not allowed.
8.3. For patients aged 15 years and older, information is provided in person.
8.4. Information regarding patients under the age of 15 is provided to their legal representatives personally.
8.5. Information is provided to the legal representatives of patients aged 15 years and older with the written consent of patients aged 15 years and older in accordance with paragraph 4 of Part 4 of Art. 13 of the Federal Act of November 21, 2011 No. 323-ФЗ “On the Basics of Protecting the Health of Citizens in the Russian Federation”
8.6. Information is provided to other persons (including grandparents) only with the written permission of the legal representatives of patients under the age of 15 years and patients aged 15 years and older personally, when identifying the person requesting information in a reliable way.
8.7. Information about the children admitted to the intensive care unit and intensive care during the first days after hospitalization is provided by the on-call anesthetist-resuscitator. Information about children undergoing treatment at the Department of Resuscitation and Intensive Care is provided by the attending physician every day at 9.30 in the lobby on the 1st floor of Building 2A.
8.8. Information about the children who were urgently admitted to the surgical department is provided by the doctor on duty the next day from 8.00 to 8.30 in the lobby on the 1st floor of the respective buildings at the location of the departments.
8.9. Information about the child’s current state of health is provided only by the attending physician from Monday to Friday, except for non-working holidays, from 15.30 to 16.00 in the lobby of the medical buildings.
8.10. In the event of a change in the place of provision of information in connection with the repair work and other production circumstances, information about the new place is communicated in the prescribed manner by the employees of the emergency department or department where the patient is staying.
9. Responsibility for violation of Internal Regulations
9.1. For violation of the Internal Rules of the Children’s State Hospital of St. Vladimir including violation of the regime of the day, the patient can be discharged ahead of schedule for outpatient treatment. In case of registration of the certificate of incapacity for work, a corresponding mark is made on the certificate of incapacity for work, drawn up by the patient’s legal representative who is caring.
9.2. If the patient, the legal representative caring for the patient, as well as the visitor, do not comply with the requirements of these Rules, the hospital administration reserves the right to limit the joint stay of the legal representative with the patient or to limit the visit to the patient or to discharge the patient for outpatient treatment if this does not threaten his life and health.
10. The procedure for resolving conflict situations.
10.1. In case of a conflict, the patient (his legal representative) can contact the department head, the department’s senior nurse directly.
10.2. On issues of violation of the rights of the patient (his legal representative), the patient and/or his legal representative is entitled to apply in writing with an appeal through the office of the hospital, or in electronic form at the email address mail firstname.lastname@example.org
